If applying for a new or existing IRA with SF Fire Credit Union, fill out this application packet. When returning to us, please designate if your IRA is new, a rollover or a transfer.
Traditional IRA: Beneficiary Designation or Change This form will allow you add, change or remove a beneficiary for your IRA account.
Traditional IRA: Change Notice This form will allow you to document changes to IRA owner information or account status.
Traditional IRA: Contribution and Investment Selection If you’d like to make a contribution to an IRA, use this form to designate which IRA product to place your funds.
Traditional IRA: Periodic Payment Changes Before Age 70.5 Use this form to select regularly scheduled IRA payments when younger than age 70.5.

Traditional IRA: Rollover Transfer Contributions Are you rolling or transferring your IRA from another institution or employer? Use this form to transfer or roll over funds.
Rollover indicates that you are bringing your IRA from one institution to another without needing a check in hand. Transferring indicates that the funds come from a financial institution in the form of a check that you are presenting.
